2) Range of services and products provided by the financial services provider, and the terms and conditions of their provision:
Business activities of PJSC "IC "UNIQA": NACE Code 65.12 "Other non-life insurance".

PJSC "IC "UNIQA" conducts insurance activities under a Licence confirmed by an Extract from the State Register of Financial Institutions dated 23 April 2024. Insurance is provided on the basis of insurance agreements concluded in accordance with the general terms and conditions of insurance products developed by PJSC "IC "UNIQA" and published on the Company's website https://uniqa.ua/insurance-products/, unless otherwise stipulated by the legislation of Ukraine.

3) Cost, pricing/tariffs, and fees (interest, remuneration) applicable to products depending on the type of financial service (for products offered to non-consumer clients, such information may be provided on an individual basis):
The cost of insurance (insurance premium) is determined by mutual agreement between the Policyholder and the Insurer based on the client's individual insurance requirements. Insurance tariffs are calculated using mathematical, statistical, and/or actuarial methods, taking into account claims history, estimated loss amounts, characteristics of the insured object, deductible amount, and other insurance conditions, in accordance with the Company's internal underwriting policy and/or the applicable tariff policy for the relevant insurance product.

4) Information on consumer rights protection mechanisms (including the procedure for out-of-court complaint resolution, as well as the existence of guarantee funds or compensation schemes applicable under Ukrainian law with respect to specific types of financial services and/or products):
In the course of providing insurance services, PJSC "IC "UNIQA" complies, in particular, with the regulatory requirements governing the protection of financial services consumers' rights.

All disputes arising under an insurance agreement between the Policyholder and the Insurer shall be resolved through negotiation, and, in the event that no agreement is reached, in the manner prescribed by the insurance agreement and applicable Ukrainian law. In the event of a complaint, clients may contact PJSC "IC "UNIQA" as follows:

In fulfilment of the Law of Ukraine "On Insurance" and NBU Resolution No. 201 dated 31 December 2023, PJSC "IC "UNIQA" complies with the requirements for maintaining insurer solvency. In addition, a guarantee fund is established from the guarantee contributions of insurers who are members of the Motor (Transport) Insurance Bureau of Ukraine (hereinafter — the Bureau), created to ensure the Bureau's solvency in mutual settlements with authorised organisations of other countries in the field of civil liability insurance with which the Bureau has entered into an agreement on mutual recognition of insurance contracts and mutual settlement of indemnity claims.

In accordance with the requirements of Clause 5, Article 7 of the Law of Ukraine "On Financial Services and Financial Companies", we hereby inform you of the following:
1) Information on the financial service — total costs, including fees, charges, and other expenses payable by the consumer in connection with obtaining the financial service, inclusive of taxes; where the specific amount cannot be determined — the procedure for calculating such costs:
Transactions related to the receipt of a financial service by an individual, including payment of the insurance premium, payment of insurance indemnity, and refund of the insurance premium upon early termination of the insurance agreement, are not subject to taxation. Exceptions include payments in respect of non-pecuniary damages, insurance indemnity to heirs, and other cases stipulated by applicable legislation, which are subject to taxation in accordance with the provisions of the Tax Code of Ukraine.

2) Essential terms and conditions of the financial services agreement, including:
a) Minimum term of the agreement:
The term of an insurance agreement is established on an individual basis in accordance with the insurance products and programmes selected by the Client (where applicable) and/or the minimum agreement durations prescribed by applicable regulatory acts.

b) Consumer's right to withdraw from, terminate, or cancel the agreement, the right to early performance of the agreement, and the procedure and consequences of exercising such rights:
The Policyholder may terminate and cancel the insurance agreement pursuant to Article 105 of the Law of Ukraine "On Insurance", and may also withdraw from the agreement pursuant to Article 107 of the Law of Ukraine "On Insurance".
Each insurance agreement includes a section setting out in detail the agreed procedure and consequences of early termination of, and withdrawal from, the agreement, which is binding upon both parties to the insurance agreement.
The insurance agreement does not provide for early performance of the agreement.
c) Procedure for amending the agreement:
Amendments and additions to an insurance agreement may only be made by mutual consent of the Insurer and the Policyholder. Any amendments or additions to the insurance agreement shall be formalised through appropriate supplementary agreements, which, upon signing, become an integral part thereof, unless the insurance agreement provides for a different procedure for introducing amendments and additions.

3) Consumer rights protection mechanisms:
a) Procedure for complaint handling and out-of-court dispute resolution:
All disputes arising under an insurance agreement between the Policyholder and the Insurer shall be resolved through negotiation, and, in the event that no agreement is reached, in the manner prescribed by the insurance agreement and applicable Ukrainian law. b) Guarantee funds or compensation schemes applicable under legislation:
In fulfilment of the Law of Ukraine "On Insurance" and NBU Resolution No. 201 dated 31 December 2023, PJSC "IC "UNIQA" complies with the requirements for maintaining insurer solvency. Працівник з реалізації страховика, у разі залучення його до реалізації страхового продукту, отримує заробітну плату, яка виплачується в порядку, передбаченому законодавством про працю згідно з внутрішнім положенням про систему управління винагородою працівників ПрАТ «СК «УНІКА».
5) інформація про будь-які інші платежі (крім страхових премій), які клієнт буде зобов’язаний сплатити у разі укладення договору страхування;
У разі укладення договору страхування клієнт не сплачує жодних платежів, крім страхових премій та комісії банку за переказ коштів на рахунок страховика.
6) інформація про механізми та способи захисту прав споживачів фінансових послуг (зокрема, про можливість та порядок позасудового розгляду скарг споживачів фінансових послуг, адресу страховика, за якою приймаються скарги клієнтів);
Усі спори, що виникають із договору страхування між страхувальником та страховиком, вирішуються шляхом переговорів, а у разі недосягнення згоди - у порядку, встановленому договором страхування та чинним законодавством України.
