<li>More than 5,000 UAH: send original documents or their certified copies by registered mail or in person to the address: 6 Oleny Telihy St., Building "V", Kyiv, Ukraine, 04112.</li>

In case of disability or death of the insured person, applications for payment are accepted within 30 days.

List of documents required to receive insurance claim payment

<li>Insurance claim form;</li>
<li>Copy of the recipient's passport (ID card) and copy of the child's birth certificate if the event occurred with a minor child;</li>
<li>Copy of the certificate of assignment of the individual tax number (ITN) of the recipient of the insurance payment;</li>
<li>Doctor's certificate, including:
<ul>
<li>document confirming the diagnosis, certified by a round or triangular wet stamp of the healthcare institution, containing necessary information including the diagnosis, medical history, and date of its initial diagnosis;</li>
<li>in case of disability: certificate from the Medical and Social Expert Commission (MSEC) confirming the disability group assignment;</li>
<li>in case of the insured person's death: notarized copy of the death certificate, copy of the medical death certificate, document specifying the diagnosis of the illness that led to death, documents confirming the grounds for the heir (applicant) to receive the insurance claim payment.</li>

Determination of the amount of insurance payment

The insurance claim payment is fixed and depends on the diagnosis, as follows:
<ul>
<li style="text-align: justify;">5-70% of the insured amount for risks such as "Illness" and "Benign Neoplasms."</li>
<li style="text-align: justify;">100% of the insured amount for risks such as "Critical Illness," "Disability of the 1st group as a result of illness," and "Death as a result of illness."</li>
</ul>

Payment timelines and mechanism

Insurance payment is made:
<ul style="text-align: justify;">
<li>To the insured's bank account or to the account of their legal representative or authorized person;</li>
<li>Via the Aval Express system. It can be received at any Raiffeisen Bank branch in Ukraine upon presentation of a passport and taxpayer identification number.</li>
</ul>
Timelines:
<ul>
<li style="text-align: justify;">Decision on payment is made within 20 business days from the receipt of the complete set of documents. If it is necessary to determine additional circumstances of the case, the term may be extended to 60 days with a reasoned notification of the reason for such a decision;</li>
<li style="text-align: justify;">Sending of funds is carried out within 5 business days from the date of decision acceptance.</li>
