<ul>
<li>full name and date of birth of the insured person;</li>
<li>contract number;</li>
<li>reason for consultation;</li>
<li>whether a sick leave certificate is needed;</li>
<li>actual address of the insured person and contact phone numbers.</li>
<li>any other information requested by the Medical Assistance specialists.</li>
</ul>
</li>
<li style="text-align: justify;">Follow the instructions and recommendations provided by UNIQA;</li>
<li style="text-align: justify;">Present a document verifying your identity (insurance card in the myUNIQA app, passport, or driver's license) during the doctor's visit.</li>
</ul>

Independent visit to the hospital

Make sure to arrange in advance with Medical Assistance for an independent visit to the doctor.
You can independently visit a hospital if it does not have contractual relations with UNIQA and independently pay for medical services at this facility, then submit the documents to UNIQA for reimbursement of expenses.
In this case, you need to:
<ul>
<li style="text-align: justify;">Contact UNIQA no earlier than ten calendar days before the date of the doctor's visit and no later than the date and time of the doctor's appointment and inform them of the date and reason for the visit;</li>
<li style="text-align: justify;">Notify Medical Assistance of any additional examinations, consultations, and/or treatment prescribed as a result of the independent planned visit within two working days from the date of appointment;</li>
<li style="text-align: justify;">Submit documents for reimbursement according to the "List of documents for receiving insurance payment" section.</li>
</ul>
If emergency medical assistance is needed, you can call an ambulance without prior contact with us. In this case, you need to inform Medical Assistance of the insurance event as soon as possible but no later than two calendar days from the date of payment for medical services/medications or discharge from the hospital.
If you visit a hospital without prior notification to UNIQA, except in the case of receiving emergency medical assistance, UNIQA reserves the right to refuse insurance payment.

List of documents required to receive insurance claim payment

<li style="text-align: justify;">Copy of the registration extract;</li>
<li style="text-align: justify;">Copy of the child's birth certificate if compensation is under the child's policy.</li>

Documents for receiving insurance payment must be provided no later than thirty calendar days from the date of payment for medical services or medications, except for cases of hospital treatment, when documents may be submitted within thirty days after discharge from the hospital.
*The mentioned deadlines may differ from the payment deadlines specified in the specific insurance contract.

Payment timelines and mechanism

Insurance payment is made:
<ul style="text-align: justify;">
<li>To the insured's bank account or to the account of their legal representative or authorized person;</li>
<li>Via the Aval Express system. It can be received at any Raiffeisen Bank branch in Ukraine upon presentation of a passport and taxpayer identification number.</li>
</ul>
Payment to the representative of the insured person is only possible with the authorization of the insured person, which must be executed in accordance with the established legal procedure. If the insured person is a minor, the recipient of the insurance payment is one of the parents (guardian). If the insured person is incapacitated, the recipient of the insurance payment is their legal representatives.
