1. Insurance Object

The object of insurance is the health of the Insured Person.

2. Insurance Risks and Limitations

Insurance Risks:

Acute illness of the Insured Person;
Exacerbation of a chronic illness of the Insured Person;
Health disorder resulting from an accident involving the Insured Person;
Other health disorders and/or cases provided for in the Program.
Insurance Limitations:
The following individuals are not eligible for insurance:

Persons aged 0 to 6 years and over 59 years;
Persons with Group I or II disabilities and persons with disabilities from childhood;
Persons officially recognized as legally incompetent;
HIV-positive individuals or those with AIDS;
Persons who, at the time of the insurance agreement, are undergoing outpatient or inpatient treatment (hospitalized);
Persons suffering from severe decompensated forms of diseases of the cardiovascular, endocrine, excretory, nervous, musculoskeletal, and other systems; or with a history of malignant neoplasms;
Persons with mental and behavioral disorders, tuberculosis, venereal diseases, alcoholism, drug addiction, or substance abuse.
A child can be insured only together with an adult.

3. Minimum and Maximum Insurance Sum (Coverage Limit)

The insurance sum is aggregate and determined by the Policyholder’s choice as follows:

UAH 75,000 under the Standard Program for one Insured Person;
UAH 125,000 under the Classic Program for one Insured Person;
UAH 175,000 under the Prestige Program for one Insured Person.

4. Minimum and Maximum Insurance Premium and/or Rate

Insurance Premium:

UAH 15,600 under the Standard Program for one Insured Person;
UAH 21,000 under the Classic Program for one Insured Person;
UAH 27,600 under the Prestige Program for one Insured Person.

5. Type, Minimum, and Maximum Deductibles (if applicable)

The product includes an unconditional deductible for consultations with leading specialists in private clinics, as specified in the Program.

No deductibles are provided for other options within the Programs.

6. Territory and Duration of the Insurance Agreement (including entry into force and periods of coverage)

Insurance Territory: Ukraine, except for:

Temporarily occupied territories of Ukraine as defined by Ukrainian legislation, territories where state authorities temporarily do not exercise their powers, and settlements located along the contact line.
Coverage within Ukraine (excluding the aforementioned areas) does not apply to war zones or any form of military actions, regardless of the formal recognition of war. Insurance covers cases unrelated to war or military conflicts, taking into account exclusions specified in the agreement.
Duration of the Insurance Agreement: 1 calendar year.

The agreement becomes effective, and coverage starts from the date specified in the Agreement as the start date, but not earlier than the day following the payment of the premium (or the first installment) to the Insurer's account, in the amount and within the period specified in the Agreement, considering the 7-day waiting period.

7. Consumer Consequences for Non-Compliance

Non-compliance with contractual obligations may result in denial of payment or cancellation of the agreement due to non-payment of premiums.

8. Availability of Standalone Purchase

The insurance product is not an add-on to other goods or services.

9. Discounts and Promotions

No discounts or promotions are provided.

10. List of information essential for assessing insurance risk and/or other circumstances considered when determining the amount of the insurance premium

The information essential for the insurer’s decision on the insurance premium amount under the contract, assessment of insurance risk, and inclusion of the Insured Person in the list of insured persons includes details on:
- the Insured Person’s profession,
- place of work,
- participation in sports,
- existing disability group,
- changes in the Insured Person’s health condition,
- active insurance contracts related to the Insured Person’s health.

When insuring a group (team of individuals), the information essential for the insurer’s decision on the insurance premium amount under the contract and assessment of insurance risk includes details on:
- employee illness rates,
- changes in working conditions that increase the risk of illness or injury,
- the presence of incapacitated or partially incapacitated persons subject to insurance,
- the Policyholder’s insurable interest in the insured object,
- active insurance contracts related to the Insured Person’s health.

For all the above-mentioned Programs, the following conditions are common:
- the object of insurance;
- insurance risks and limitations;
- the territory and duration of the insurance contract [including information on its commencement and insurance periods (if applicable)];
- possible consequences for the consumer in case of failure to fulfill obligations defined by the insurance contract, including late notification of an insurance event without valid reasons and late payment of the insurance premium or its subsequent installment;
- information on the possibility of purchasing the insurance product separately if it is offered together with an accompanying and/or additional product, service, or work that is not insurance-related, as part of a package or contract;
- conditions for receiving a discount on the insurance product and promotional offers from the insurer (if available), including their validity periods.
