General terms and conditions of the insurance product “My Health” (code 1100), as amended on June 30, 2026

1	Object of insurance	The object of insurance is the health of the Insured persons.
2	Insurance risks. Insurance limitations	Insurance risks: acute illness of the Insured person; exacerbation of chronic illness; health disorder due to an accident; other cases specified in the Program. Limitations: persons over 70 years old.
3	Minimum and maximum insured amounts	Not limited. Determined by agreement and specified in the Program. Amounts and limits are aggregate — reduced after payments.
4	Minimum and maximum premium/tariff	UAH 1,380 – UAH 150,000 per person. The exact amount is specified in the contract.
5	Deductible	Specified in the Program: in UAH or percent. If in percent — calculated from the loss amount.
6	Territory and term of the insurance contract	Territory: Ukraine, excluding occupied and uncontrolled territories, contact line. Coverage in war zones applies if not war-related. Term: 1 calendar year (unless otherwise agreed). The contract takes effect from the start date specified in the contract.
7	Consequences of non-performance of obligations	Payment may be refused. If the premium is not paid, the contract is deemed not concluded (unless otherwise agreed).
8	Separate purchase	The product is not an add-on to other goods or services.
9	Discounts and promotions	Not provided.
10	Information of material importance	profession; workplace; sports activities; disability; health changes. For group insurance: employee morbidity; working conditions increasing risks; presence of incapacitated or partially incapacitated persons; policyholder’s insurable interest; existing contracts.
11	Consumer notice	The consumer must review terms, exclusions, and payment rules.
